Summary:
|
Mountain View Los Altos High School District is requesting Change Order #4B with Galeb Paving, Inc. be ratified:
- Spoils off haul that was generated by others and left in the work area of Galeb. Also, demolition limits were increased per asphalt removal as noted.
- Reclamation of winterization materials and re-excavation of bio retention planter areas.
- Repair broken sanitary sewer line caused by Sausal on 7/9 in order to maintain scheduled concrete pour.
- Excavate silting basin per construction manager's direction onsite.
- Import of topsoil indirectly caused by request for information 453 response, and Department of Toxic Substances Control (DTSC) requirements. Topsoil added was placed in the planter area, and orange fence material was added under the subgrade.
- Remove and replace asphalt where the project trailer was located. Added asphalt was due to previous patching by others and existing areas. The cost noted is to remove all of the areas noted and replace with new asphalt.
