Summary
:
Mountain View Los Altos High School District is requesting approval of the agreement with Uplift Family Services to provide 1 Full-time Social Worker who will primarily provide case management services for students eligible. Additional services may include school/parent presentations. These social work services in schools including case management, therapy, and community resourcing and linkage may be delivered in the school or in the community as appropriate for the specific youth.
