Summary:

Mountain View Los Altos High School District is requesting Change Order #2C with Robert A. Bothman be ratified:
- Added scope to pothole, trace the existing gas line, excavate, and reconfigure a section of existing 6” gas main line. The revisions are required due to the gas line being in conflict with the new building footings shown on the plans.
- Using $4,000.00 of Allowance C1 (temporary water and sewer connection), to stub-up existing domestic water source to supply construction water to site for concrete wash-out, and site water to keep dust down during excavation of the project
- Remove existing shrubs at East drive property line, per District request. This was not shown to be removed on current DSA approved plans. Shrubs were in the way of the construction utilities and job trailer.
- Backfill joint trench with 230 Lineal feet of concrete slurry
from bottom of footing to subgrade, per RFI directive due to updated storm drain inverts and relocation. The 10” storm drain line was relocated to maintain an access path for the students at the 200 Wing Building for the school year. Cost includes slurry seal for storm drainpipe and sanitary sewer line.
- Existing 6” domestic water supply is not as plans denote requiring relocation where it’s using an open joint trench. Added cost covers time and material with equipment to relocate the existing 6” domestic water line. The existing open trench will be utilized to save costs.
- Additional work due to an existing water line leak that needed to be sourced. Contractors found that the existing restraint system had a proficient leak which was existing for some time and went
unnoticed due to being very deep underground. The contractor removed the restraint and replaced it with a new more modern mega lug system.
- Excavate and repair a leak on domestic water line caused by vibration and compaction on the building pad. The leak was after the fact and not directly tied to a certain contractor who compacted the area, likely causing the leak.
