Summary:
|
Mountain View Los Altos High School District is requesting Change Order #6C with MK Pipelines, Inc. be ratified:
- Furnish and install additional 240 lineal feet of 3" water
lines.
- Cost for Work Directive #30 was projected to be under
$10,0000; however, additional expenses were incurred
to lower utilities for lime treatment on T&M.
- Install new catch basin and pipe due to added curb near
planting area.
- Due to condition and location of an existing pipe, a
pressure test and backfill of trench was required for
proper tie-in of existing line.
- Install retaining wall with catch basin due to relocated
bio retention wall footings within classroom courtyard
and Tech building.
- Reconstruction of existing catch basin in planter area
and catch basin moved to new location.
- Raise and install rainwater leaders from downspouts to
collection boxes for all gutters and downspouts.
- Fix existing downspouts and install new ones, to divert
water from classroom building away from construction
zone. Existing sewer clean-out box dug out at higher
elevation to divert storm water from entering.
- Install 4 yards of slurry at end of bore pit to fill void
around 12' pipe.
- Repair sewer leak from an existing pipe, which was
leeching in native soil.
- Repaired broken 10” water main pipeline with C900 pipe
replacement and mechanical joint sleeves installed.
