Summary:
|
Mountain View Los Altos High School District is requesting Change Order #1K with Casey Fogli Concrete Inc. be ratified:
- Revised costs from bid set to DSA approved set changes to the plans.
- Expedited overtime to meet schedule deadline of the first day of school start, per District request.
- Added folding bollards behind the gym building, per District request to limit access to new service road not on bid set plans.
- Resurface and patch existing stairs and adjacent landing area at door entry of locker room building to match adjacent new areas recently renovated, per District request. The existing concrete area was in disrepair and required repair to remove defects on the concrete surface.
- Form, place, and finish approximately 250 lineal feet of trench drain per District request. This was added along the fire truck lane behind the new gym and Los Altos Field area.
- Added top cast protection at interior walkway at classroom building to protect new concrete from scaffolding yet to be installed, per District request.
- CCD #85 stair way revisions due to changes in the concrete landing area. This is at the south stairs near the courtyard.
- Supply and pour added concrete behind planter wall cavity directly behind seat wall, per RFI 439 and District request.
- Revisions in cost for turf under the stair areas from the bid set to DSA approved plans.
