Summary:

Mountain View Los Altos High School District is requesting Change Order #6C with Robert A. Bothman Construction be ratified:
- It was discovered the district warehouse is not being supplied water from Bryant Street and is instead being fed from the campus. In order to safe-off water to demolish the film classroom and still supply the warehouse, there will need to be a temporary re-route of the water line until the permanent water line is installed and connected to the buildings. Provide labor and materials to re-route and install the water line which will supply the district warehouse, until the permanent water is installed, as directed by Request for Information #29. This will be done on a Time and Materials not to exceed basis.
- Provide labor, materials, and equipment to excavate a deep trench, install roughly a 20’ length of 12” storm drain, refill and compact so that the new storm drain system can be tied into the existing point of connection outside the limits of construction. The original point of connection for the storm drain was shown on the plans but was discovered that the actual storm drain connection was outside of the school limits in the property of the city. This work will be done on a Time and Materials basis.
- Provide labor and materials to provide (8) total storm drain drop inlets (drainage box) that were omitted from the plans. The civil plans have a note that directs the contractor to stub and cap 4" Storm Drain for landscape drainage, and it directs to landscape plans for continuation. But when reviewing the landscape plans the drop inlets are not shown on the material plan and there is no detail shown on the plans. Civil engineer reviewed and directed to add 12”x12” drop inlets so that there is proper drainage in the planter boxes.
