Mountain View Los Altos High School District is requesting Change Order #11A with Beals Martin be ratified:
- Wood flooring manufacturer requires a certain flatness/levelness, for the concrete slab, to install flooring material. When the concrete slab was poured, initial testing was performed to ensure flatness and levelness is accurate, however, after a few months, in some locations, concrete slab has curled (bends as it dries up). It's unforeseen conditions. Grind and fill (using a self-leveler underlayment) to prepare slab for flooring material. Original T&M WD 055 was $54,384.00, and work has exceeded the budget, this cost is in addition to previous budget.
- Due to sequence of construction activities, we had to relocate the construction site entrance and demo about 35 linear feet of existing fence. Now that construction is completed, furnish and install 35 linear feet of chain link fence and connect to existing fence. |
