Summary:
Mountain View Los Altos High School District is requesting Change Order #4A with Beals Martin be ratified:
- Material price increase for overhead coiling (3) & sectional/vertical (2) doors (both are roll-up doors, different in installation) from bid day to fabrication. Per contract documents, contractor is responsible for 6% of unforeseen increase and District is responsible for the rest. Cost includes 3 key control (electronic switch to open/close doors) surface mounted switches for coiling doors not included in original plans.
- Provide gym floor lettering and spartan logo, per District request, not part of bid documents.
- Due to field conditions, metal plates were required to support mechanical units; Contractor furnishing and install of 5 plates at deck flute.
- Provide galvanized metal flashing (sheet metal) at edge of gym roof to connect metal decking to the beam.
- For safety and better access to the overhead coiling door motor, contractor install of internal tube type motor located above door, in lieu of the original motor located on the side. Cost is for all 3 coiling doors located at the gym storage.
